FinalForms Instructions

You will need the following information to allow for accurate completion of your online registration:

• Basic Medical History & Health Information
• Insurance Company & Policy Number
• Doctor & Dentist Contact Information
• Hospital Preference

Creating an Account

Parents are required to create the Final Forms account. Select *New Account and follow the Registration Steps. If you need assistance creating your account, please contact FinalForms technical support at support@finalforms.com.

Registering a Student

1. Visit FinalForms
2. Click Login under the Parent icon.
3. Click Incomplete Forms.
4. Select all the sports for which you would like to register. The selection(s) can be changed at any time prior to the registration deadline.
5. Complete each form and sign your full name in the Parent Signature field at the bottom of the page. After signing a form, click Submit Form and move on to the next form.
6. You will see  a “Forms Finished” message when all forms are complete.
7. If required, an email will automatically be sent to the your student, prompting them to complete the forms requiring their signature.
8. Click MY STUDENTS when you are done. Repeat steps 3 – 6 for additional students.
9. If you need to update your information, click the Update Forms button.

How do I upload a document?

1. Go to FinalForms
2. Log in, and go to My Students page.
3. Click the Folder in your student’s row.
4. Use the popup box to upload a document.

Insurance

Medical insurance is required to participate in high school athletics.

If you do not have medical insurance, we provide information regarding student accident and health insurance coverage administered by Myers-Stevens & Toohey, Inc. Coverage is available for:

• Interscholastic Tackle Football
• School-Time Accident Plan
• Full-Time 24/7 Accident Plan
• Dental Accident Plan

These low cost plans are used by many school districts across the state. Although they are not the only provider of this type of coverage, they have been recommended by our Washington Schools Risk Management Pool.

Physical Participation Forms

Up-to-date Physical Forms must be on file with the Garfield Athletic Office for participation clearance. Physicals are current for two calendar years.

Don’t know if the physical is current? Login to The Source if you were enrolled in a sport at Garfield during the 2021-22 school year.

A current copy may be uploaded into an athlete’s Final Forms account for clearance. Emailed copies of physicals are not accepted.

Fees

Students participating in Garfield Athletics are required to pay a participation fee (ASB). These funds directly support the cost of athletic programs: game transportation, equipment, uniforms, and more. Athletes must provide proof (receipt) for final athletic clearance. To securely purchase ASB for the 2022-23 school year. Upload a copy of the SchoolPay receipts to your FinalForms account.

Students Not Enrolled Full-time at Garfield

Students are required to complete a WIAA contract if they do not attend classes on campus at Garfield full-time.

Running Start

If you are enrolled in Running Start, please complete the WIAA Running Start Contract on the WIAA website. To find it, click Expand below Student Eligibility and Related Issues  and then scroll down to find the WIAA Running Start Contract .Download a copy of your Running Start class schedule. Upload the Running Start class schedule and contract to your Final Forms account.

Off-Campus Classes/Alternative Schools

If you are enrolled in any off-campus classes, please complete the Alternative Education School Contract on the WIAA website. To find it, click Expand below Student Eligibility and Related Issues and then scroll down to find the Alternative Education School Contract .Download a copy of your class schedule and upload the schedule and the Alternative Education School Contract to your Final Forms account.Depending on your status please follow the instructions below. IMPORTANT: You must complete the two steps below prior to uploading documents to your Final Forms account.

1. Confirm your neighborhood school by using the “Address Look Up Tool”. Go to schoolpay.com and purchase ASB for Garfield. Upload the Schoolpay receipt to your Final Forms account.

Grades

Grade checks are run throughout each sports season. Please upload a copy of previous semester and most recent quarter grades to your Final Forms account. Failure to provide grades may make you ineligible for competition.If you attend any other school for ANY classes, you must provide the Athletic Department with your grades from that school.

Students Not Enrolled Full-time at Garfield “Out of District”

Students are required to complete a WIAA contract if they do not attend the school full-time. All Documents must be uploaded to FinalForms.

Off-Campus Classes/Private Schools & Home School

If you are enrolled in any off-campus classes, please complete the Alternative Education School Contract on the WIAA website. To find it, click Expand below Student Eligibility and Related Issues and then scroll down to find the Alternative Education School Contract .Download a copy of your class schedule for your school. Upload the class schedule and Alternative Education School Contract to your Final Forms account. Depending on your status please follow the instructions below.

Home School

If you are home schooled, please complete the Home Based Education School Contract on the WIAA website. To find it, click Expand below Student Eligibility and Related Issues and then scroll down to find the Home Based Education School Contract .Upload your completed “Home Based Education School Contract” to your Final Forms account. Important: All “Out of District” athletes must also follow the instructions below prior to bringing documents to the Athletic Department.

1. Confirm your neighborhood school by using the “Address Look Up Tool”. Guardian/student completes SPS Online Enrollment Once enrolled, please contact the Athletic Assistant at 206-252-2279. (Please do not email.)Upload current physical forms to your Final Forms account. Purchase ASB for Garfield. Upload the Schoolpay receipt to your Final Forms account.

Grades

Students who transfer to Garfield must complete the  WIAA Eligibility Packet (linked).  Select “New Student Athlete” and follow Step 1 and complete a New Student Packet. Bring the completed form along with a current transcript to the Athletic Director.  The form required for all students transferring to Garfield who want to participate in a sport.  

NOTE:  All transfer student athletes from Private High Schools to Garfield and student athletes seeking a 5th year of athletic eligibility are required to have an eligibility hearing with a SeaKing District 2 hearing committee before being cleared to participate in an athletic contest. All SPS academic eligibiity rules apply to transferring students.

FinalForms is an online Athletic forms used for Garfield High School Athletic Registration. FinalForms allows you to safely and securely complete and sign athletic participation forms from your computer, tablet or mobile device.

FinalForms saves data from season-to-season and year-to-year, making the registration process quick and easy. Athletes may register for multiple seasons at the same time. Additionally, users may login any time to update information including emergency medical contacts and information.  Users will be required to sign forms once per year and anytime an update is made.
