Fall Sports Physical Days

Garfield Teen Health Center | Aug. 20 – Sept. 2, 2026 | By Appointment

The Garfield Teen Health Center will reopen August 20, 2026 and offers sport physical exams for SPS students for no out-of-pocket expense to families (Labs processed outside of the clinic may incur a fee). Sports physicals are also available by appointment throughout the school year.

What is a Sports Physical?

A sports physical exam is required before student-athletes can tryout or participate in school sports. The purpose of the exam is to ensure that students can safely participate in school sports and help prevent sports injuries. Sports physicals expire every 2 years. During the visit, a medical provider will check your vital signs, vision, and do a brief physical exam.

  • Sport physical exams are required for students to tryout or participate in school sports. 
  • SPS Sport Physical forms expire every 2 years.
  • There is no out-of-pocket cost for sport physical exams at the Teen Health Center.
  • The Teen Health Center does not offer drop-in appointments; families must schedule in advance.
  • Parent/guardian consent is required for sport physical exams however, guardians are not required to be present for the exam.
  • If a student has had a well-child check with their primary care provider (PCP) within the past year, the PCP may be able to complete the required forms without an additional exam. We encourage families to check with their primary care office before scheduling an appointment with GTHC.

Consent & Sports Physical Forms

The following forms must be completed by a legal guardian prior to the appointment: 

  1. Guardian Consent Forms: Complete online.
  2. SPS Sport Physical Form: Available to complete online.

The Odessa Brown Children’s Clinic at Garfield High School offers medical and behavioral health services to make care convenient and accessible for students and their families. The goal is to support student success at school and beyond by providing care where they spend most of their time — at school.
