What is the Teen Health Center?
The Teen Health Center (THC) is a safe, confidential place where students from Garfield and Nova High School can receive medical, nutritional services and mental health care. The Teen Health Center promotes how to live healthier lifestyles through education and counseling. All services are free of charge for students.

How can you get services at the Teen Health Center?
Students must be registered with the THC and have a scheduled appointment before receiving services.
A parent or guardian must sign a consent form for a student, for services such as:
- Sports Physical Exam (page 1 of this form must be completed at appointment time)
- Sports Injuries
- Immunizations
- Nutrition Information
- Academic Concerns
Garfield Teen Health Center consent for comprehensive services.
Students can sign for themselves in certain cases where services are provided confidentially such as reproductive health concerns and emotional/mental health services. A Minor Consent will be need to be completed in person.

The Garfield Teen Health Center requests to see school records so that they can work with Garfield and Nova staff/teachers to improve student learning. By signing the FERPA forms (provided in the THC), this gives staff permission to see school records to help facilitate care. Please sign and return the FERPA forms with the full registration consent form.

Immunizations
As part of the continued effort to keep Garfield and Nova students safe and healthy, we work closely with the school nurse and parents to ensure kids are up to date with their vaccinations.
Students need to have a full parental consent and a parent-signed Vaccine Informational Sheet in order to receive any vaccination.
Students can visit the THC anytime to make an appointment, and may be seen promptly if a provider is available.
You can print and sign the form below for the vaccines needed. You can also e-mail the Nurse Practitioners (contact information listed above) and we can contact you via DocuSign secure messaging to obtain your consent.
