    Student-Athlete Forms

    Welcome to Garfield Athletics! Please use the following information to register to play a sport at Garfield High School. Please print, complete, sign and return forms, show proof of ASB purchase (sticker on ID or receipt from SchoolPay) to the Athletic Office (Room 122). Incomplete registration packets may not be submitted.

    Spring Sports Begin Monday, February 26, 2018

    Completed Athletic Registration packets for Spring Sports are due by 4 p.m. on Wednesday, February 21, 2018 in order to participate on the first day of tryouts, Monday, February 26th. If you participated in a Fall or Winter sport, two forms must be completed and submitted:
    1. GHS Athletic Eligibility Information
    2. Sport Specific Guardian Release Form (See Step 2 below)

    Paperwork will be accepted on Monday, February 26th to participate in tryouts on Tuesday, February 27th.

    Step 1: Student Athletic Participation & Registration Forms

    Please complete on-line, print and return the forms below to the Athletic Office (Room 122), before school or during lunch.

    • Athletic Registration Forms
    • Physical Participation Forms (if you need just the physical form to take to doctor's office - the form is included with the Athletic Registration forms above)
    • Running Start Form - FOR RUNNING START STUDENTS ONLY (Needs to be completed and turned in with Athletic Registration Forms. Please include a copy of your Running Start schedule)

    (Note: Registration forms valid for one school year only; physical forms valid for 24 consecutive months. Sports Physical Expiration date has been added to the Source - it is located in a box at the top of the screen.)

    Step 2: Sport Specific Guardian Release & Warning Forms

    For each school year, one form required for each sport student will participate.

    Step 3: Guardian Awareness Forms

    These three (3) forms - Transportation Form, Weight Room form and Concussion and Sudden Cardiac Arrest Awareness form are included in the packet above.
    The Concussion & Sudden Cardiac Arrest Awareness information sheets should be read and referred throughout year, but do not need to be turned in.

    Step 4: Enrollment Forms (if applicable)

    This checklist and all required forms must now be submitted online directly to the District Office.

    To begin registration, parent/guardians must create an InfoSnap account using your email address. Click here to access the registration link:

    You will also need to upload the following documents:

    1. A signed Certificate of Immunization Status (CIS) form;
    2. Photo ID
    3. Two address verification's which contain parent/guardian name (example -utility or cable bill,or lease),birth certificate or passport showing student's birth date).

    Make sure your student is in Garfield's attendance area by utilizing the Address Lookup Tool.


    Please email Leslie Hurd ( after you have uploaded the enrollment information and all supporting documents. We will notify you once we receive confirmation that the cross-enrollment has been completed.

    Step 5: Purchase ASB Sticker and Clear All Fines

    To purchase ASB and/or clear fines visit  Student ID with the ASB sticker or receipt from SchoolPay, verifying payment, are required when turning in completed sports registration packets.

    Step 6: Turn in Forms to Athletic Office (Emailed copies are not accepted)

    Bring originals of completed and signed forms, along with proof of ASB to the Athletics Office (Room 122). Students may only turn in forms before school, during break, lunch, or after school. We look forward to meeting them!  GO BULLDOGS!
