    Start of School Forms

    Back to School Forms

    As we prepare for a new school year, we ask that families verify or update contact information, select notification preferences, agree to guidelines on acceptable use of technology, etc. via start of school forms. These required forms are now available on the district's website. Please make plans to complete these important forms before the start of school so staff have the most up-to-date information for your student. 

    Access all the required start of school forms on the SPS website and additional information/forms that may be applicable to your student.

    Please ensure that the following forms are completed and returned:

    • FERPA/Opt-Out Form
    • 504-2 Survey 
    • Emergency Information and Student Release
    • Student Housing Questionnaire
    • Indian Education Program, Title VI
