PE Waiver

Physical Education Waiver and Log

  • Directed Athletics – Coach verification must be on letterhead.
  • Employment – Employer verification must be on letterhead.
  • Other Good Cause (Formerly known as Academic Waiver) – Transcript must show 5 semesters of passing coursework.

Due Dates:

  • First Semester – March 1
  • Second Semester – October 1
  • Senior Final Waiver – June 1

PE Waiver FAQ

PE waiver forms are available on the Counseling website. Deadline for 1st semester is March 1st. Deadline for 2nd semester is October 1st. If you are a senior your final waiver is due June 1st. Please make a copy (or take a photo) of the waiver prior to turning it in. Completed forms go to your assigned counselor who will forward it on to the Principal for approval.

The activities must include regular practices, and some form of competition, totaling at least 80 hours. Your coach must sign your PE waiver form. If you have questions or uncertainty about your particular activity being approved, please check with your counselor prior to starting your activity.

This would fall under the “category 6- Other Good Cause”. Students must have a full academic schedule – no Teachers Assistant courses, Early Dismissals or Late Arrivals – for all 4 years. Students can start to apply for this waiver beginning 2nd semester of 11th grade. If they have completed and passed the Personal Fitness class. The alternative option is to take the PE Competency Exam which can be taken by contacting your counselor.

If playing a Garfield WIAA sanctioned sport students must submit a PE waiver form to their assigned Counselor signed by a Garfield Coach verifying that the student has participated for the entire season in practices and contests. At the end of the season, coaches will provide a list to the Athletics Director. Athletics Director will notify the Registrar of qualified students. The Registrar will then make the posting to the student’s transcript. No credit is awarded, but if approved, it will show on the student’s transcript as “HPE8205 PE W-DIR ATHLET P 0.00 0.00”
